(BEDFORD) - Free sports physical exams will be available for all current 8th-, 9th-, 10th- and 11th-graders in the NLCS system on Wednesday, May 15 starting at 6 p.m. in the BNL auxiliary gymnasium.
This year a parent or guardian must be present with their student-athlete during their physical. If a parent/guardian is not present, the athlete will not be allowed to get a physical.
To receive physicals, a physical form must be filled out completely. Parents must complete the medical history and insurance information sections of the form. This physical is for the 2013-14 athletic seasons, so the grade marked on the box should be what the student will be in for that year.
The parent and athlete must sign the form in two different places where provided, and the forms must be returned to the BNL athletic office or athletic training room no later than Monday, May 13. This will reserve the athlete's spot for a physical, and no walk-in exams will be allowed the day of the physicals.
