Anna Rochelle, of the Greene County Daily World reports, Julie A. Richardson, 41, was arrested on April 29 after a warrant was issued for her arrest on April 16.
The offense allegedly occurred last fall, but a toxicology report didn't arrive back until early April.
On October 7, Indiana Conservation Officer Matt Landis stopped Richardson after he observed her driving at an extremely slow speed, weaving in and out of her lane of travel.
When she was stopped, the officer reported her speech was slurred and her eyes were bloodshot and glassy.
She allegedly failed standard field sobriety tests and registered .04 on a portable breath test. She did pass the alphabet test.
Richardson submitted to a blood draw and a sample was sent to a state lab.
When the results were returned on April 2, the analysis report indicated she tested positive for methadone, carisoprodol and meprobamate.
Methadone is classified as a Schedule II controlled substance while meprobamate and carisoprodol are classified as Schedule IV controlled substances.
When ICO Landis submitted a probable cause affidavit, he confirmed that Richardson was also convicted of operating a vehicle while intoxicated on Aug. 31, 2011.
Richardson's bond was set at $8,000 surety with ten percent cash allowed. She was released the same day as her arrest after posting $800 cash.
When Richardson appeared on Monday in Greene Superior Court for her initial hearing, she was charged with: Operating a vehicle while intoxicated endangering a person, a class A misdemeanor; operating a vehicle while intoxicated with a prior conviction, a class D felony; operating a vehicle while intoxicated, a class C misdemeanor; operating a vehicle while intoxicated with a prior conviction, a class D felony; operating a vehicle with a Schedule I or II controlled substance, or its metabolite, in a person's body, class C misdemeanor; and operating a vehicle with a Schedule I or II controlled substance, or its metabolite, in a person's body with a prior conviction, a class D felony.
The court entered an order recommending an immediate suspension of Richardson's driving privileges.
