(HUNTINGBURG) - A woman has been charged with six felony counts after she worked under a fraudulent name, had a fake driver’s license and used a fraudulent identification at a Dubois County hospital.
The insurance fraud investigation dates back to the summer of 2012.
Huntingburg police say 42-year old Berenice Barrientos-Galeana is facing six felony counts of synthetic identity deception and is being held at the Dubois County Security Center.
She was arrested after police discovered she was using a fraudulent name - Sarah Ann Hernandez.
She also had used a fraudulent birth date and was driving with a fake Indiana driver's license.
The furniture company's insurance carrier was notified after she sought treatment at Jasper's Memorial Hospital and they discovered she was using a fraudulent identification.
Galeana was arrested Tuesday night after she was questioning by Huntingburg and Dubois County detectives.
She is being held on bond in the Dubois County Security Center.
