INDIANA – The Division of Aging has updated its email addresses with the intent to better serve all inquiries more efficiently.
Going forward please direct inquiries to the most appropriate email listed below.
- Interest in becoming an Aged & Disabled Waiver, Traumatic Brain Injury Waiver, or Money Follows the Person provider.
- Applications may be submitted to the Division of Aging provider applications email: Daproviderapp@fssa.IN.gov
- Information about becoming a waiver provider may be found on the Division of Aging webpage, by clicking here.
- Active Aged & Disabled Waiver, Traumatic Brain Injury Waiver, or Money Follows the Person providers needing to update your profile.
- This may include but is not limited to adding or removing services, adding or removing counties to serve, change of ownership, or addressing changes.
