SEYMOUR – Schneck Medical Center requests your thoughts on the community’s health needs.
The hospital is launching a Community Health Needs Assessment and is inviting anyone in the community to participate.
The confidential survey will be used to understand the community’s most pressing health issues and wellness concerns and help Schneck plan and allocate resources to serve community needs better.
You can take part in the survey using this link, or by picking up a physical copy at the greeter’s desk from 7:00 a.m. until 4:00 p.m. Monday through Friday.
The deadline to take the survey is Friday, August 16th.
